| Welcome Letter |
The Rock Bridge 4-H Club welcomes you! Please read our welcome letter!

Enrollment Info
|
Each member enrolls in the club and in the projects of their choice.
The following forms are required to complete enrollment: 1) New or Returning
Member Enrollment Form 2) Member Health Form; 3) Code of Conduct Form;
4) Shooting Sports Waiver and Health Form for each discipline enrolled.

| Shooting Sports Waiver |
Each member who enrolls in a shooting sports project must have a
health form and a waiver signed for each discipline the member is enrolling in. Example:
If the member enrolls in archery and shotgun, they must have two member health forms and
one shooting sports waiver completed. Reason: Each discipline leader must keep a copy of the
signed health form with them during project meetings.
